When doing a monogram for a couple, you use the married last name as the initial bigger in the middle. The brides first name initial would be the first letter in the monogram.  The grooms first name initial would go on the right side of the last name initial.  For example Sara Rogers is marrying Tom Moore so you would combine them to get sMt.  Moore would be the last name bigger in the middle.  You can use either the male or females first name initial first but it is more popular to use the females first.
